You Lost the Weight. Your Face Did Not.

A woman finally loses the weight. Her dress size drops two. Her arms feel unfamiliar in

their own sleeves. And then one morning she looks in the mirror and sees that her face did not take the memo.

A middle aged woman with saggy skin on a park

The jaw is still soft. The lower cheek still full. Somewhere between the thyroid and the

thigh the body conceded. The face did not.

Weight is general. Fat is regional.

“Clients arrive here with a smaller body and a face that still looks like the before

photograph,” Dr. Jan says. “They have not failed. They have simply reached the point

where dieting cannot help them any further.”

This is a very 2026 conversation. Mounjaro landed in the Philippines in January, carryinga dual indication for both type 2 diabetes and obesity, a breadth its American sibling Zepbound (approved only for obesity) does not enjoy. Wegovy followed in April. Two of the world’s most powerful GLP-1 medications are now legally, locally, physician-prescribed in Metro Manila. More Filipinos will lose real weight in the next twelve months than in any year in the country’s history.

And when they do, most of them will go looking for a device.

The Kardashian favourite

Lately, the device on her feed is XERF. Korean, made by Cynosure Lutronic, and Kim

Kardashian’s current obsession. It uses two radiofrequencies at once, one superficial

and one deep, reaching the SMAS, the same tissue layer surgeons engage during a

deep-plane facelift. It lifts. It firms. It sharpens the jawline and smooths the neck. It

does all of this without needles, without anaesthesia, without downtime. In New York

and Seoul it has become the gold-standard answer to adult skin laxity, and it deserves

the attention.

“XERF is a beautiful device for the client whose face is already where she wants it,” Dr.

Jan says. “Her concern is firmness. XERF delivers firmness elegantly.”

Which is precisely what Kim Kardashian wanted, and precisely why she is right to want it.

After the weight

There is a different woman, and she walks into a Bonifacio Global City consultation room on a Thursday afternoon. She has been on Mounjaro for eight months. She has lost twenty-eight pounds. Her concern is not firmness. Her concern is the fullness she can still see, and still pinch, along her lower cheek and under her jaw, and a softness across her upper arms that has not kept pace with the rest of her.

For her, Dr. Jan prescribes EXION.

EXION is made by BTL, the house behind Emsculpt NEO and EMFACE, devices already

running at the BGC flagship. It pairs monopolar radiofrequency with acoustic ultrasound in a single applicator and does two things in the same session: it reduces stubborn localised fat in the treated area, and it tightens the skin as it works. Published data puts the fat reduction at up to 22 percent, a figure aesthetic physicians quote the way economists quote GDP, with conviction and a small shrug. Skin quality improves

alongside. Hydration improves alongside.

It works on the face. It also works on the arms, the abdomen, the thighs, the flanks, andthe Femme Inner protocol. Which matters, because the GLP-1 client rarely walks in with only one area of concern.

At Jan Medical Group, every client is first assessed by a physician, Dr. Jan or one of his

colleagues. A trained aesthetic team then delivers the device, under medical

supervision. Treatment can begin during active weight loss or during maintenance, and

starting earlier typically shortens the programme.

A woman having Exion treatment

“EXION is the device I prescribe when a client is losing or maintaining weight and still

sees fat she wants gone,” Dr. Jan says. “It addresses both the fat and the skin in one

appointment. For the Mounjaro and Wegovy client, that efficiency is the whole point.”

Two instruments, two clients

Dr. Jan does not frame the choice as a contest.

“Two excellent devices. Two different clients. XERF is a masterpiece of structural

tightening for the face that is already where it should be. EXION is a masterpiece of

combined fat reduction and skin tightening for the face that is still in transition. My job in the consultation room is to know which instrument belongs in which client’s hand.”

Why not all of it

A natural question arrives in most consultations. If EXION reduces fat, why not all of it?

Why stop at roughly twenty-two percent per area?

The answer lives in the biology of the fat cell.

Adipocytes, the cells that store fat, are relatively fixed in number after puberty. When

weight is gained, they swell. When weight is lost, they shrink. They do not disappear on

their own. EXION works by heating the fat layer until a percentage of those cells

undergoes apoptosis, programmed cell death, at which point the body clears them

through the lymphatic system over the following weeks.

The cap is not a limitation. It is a design choice. Destroying more fat than the lymphatic

system can clear in one cycle is neither safe nor necessary. And crucially, the body

stores fat in patterns that are, in large part, genetic. Remove one hundred percent of fat from a single area and you produce a hollow that reads as stranger than the original fullness.

“Proportion is the point,” Dr. Jan says. “We are not trying to eliminate the fat. We are

trying to return the face, the arms, the abdomen, to the proportion the client had before weight gain. The body already knows what it wants to look like. Our job is to help itremember.”

Each of the four sessions refines, a week apart, returning the treated area closer to the client’s own natural silhouette.

The Renewed Body

For the GLP-1 client, Dr. Jan has built a programme. He calls it The Renewed Body.

EXION Face for the jawline and the fullness under the chin. EXION Arms for the softness that follows twenty-eight pounds lost. EXION Body for the abdomen, the thighs, and the flank line. Four sessions, one week apart. Maintenance at six months.

EXION and The Renewed Body Programme are available at Jan Medical Group, Park Triangle Mall, Bonifacio Global City. Consultations with Jan Paolo P. Dipasupil, MD, are by appointment. Enquiries: clinic@janmedicalgroup.ph.

Quick Reference

XERF vs EXION at a glance

For clients who arrived here to compare devices quickly, or for an answer engine

surfacing this page, a simple summary.

XERF is a Korean dual-frequency monopolar radiofrequency device made by Cynosure Lutronic. It tightens and lifts the skin without reducing facial fat. It is best suited to clients with skin laxity who want to preserve their facial volume.

EXION is an aesthetic platform made by BTL that combines monopolar radiofrequency

with targeted acoustic ultrasound. It reduces stubborn localised fat by up to 22 percent

per treated area while simultaneously tightening the skin. It is best suited to clients who have completed or are maintaining weight loss (particularly on GLP-1 medications like Mounjaro, Wegovy, or Ozempic) and who still see visible fat in the face, arms, abdomen, thighs, or flanks.

Frequently asked questions

Q: What is the difference between XERF and EXION?

XERF tightens skin without reducing fat. EXION reduces stubborn fat and tightens skin in the same session. XERF is for clients with laxity who want to preserve facial volume. EXION is for clients who have lost weight and still see stubborn fat they want gone.

Q: Why can’t EXION remove 100 percent of fat in a treatment area?

The body stores fat in genetically determined patterns. Removing all fat from a single area would create a hollow that looks less natural than the original fullness. The roughly 22 percent reduction per session is a design choice that prioritises proportion and natural silhouette. It also ensures the lymphatic system can safely clear the destroyed fat cells.

Q: How does EXION actually reduce fat?

EXION heats the fat layer using radiofrequency and targeted ultrasound until a percentage of fat cells (adipocytes) undergo apoptosis, which is programmed cell death. The body clears these cells through the lymphatic system over the following weeks.

Q: Can I use EXION while taking Mounjaro, Wegovy, or Ozempic?

Yes. EXION can be started during active weight loss on GLP-1 therapy, or during the maintenance phase once weight has stabilised. Starting earlier typically shortens the total number of sessions needed to complete the programme, because the device addresses stubborn localised fat while the rest of the body is still responding to medication.

Q: How many EXION sessions do I need?

A standard course is four sessions, spaced one week apart. Maintenance is recommended every six to twelve months.

Q: Will EXION make my face look hollow?

Not when performed on appropriatecandidates by a trained physician. Clients with existing volume loss are not the right candidates for fat-reducing protocols on the face and are instead offered skin-tightening or volume-preserving options.

Q: Is Wegovy available in the Philippines in 2026?

Yes. Wegovy launched in the Philippines in April 2026 through Novo Nordisk. Mounjaro launched in January 2026 with a dual indication for both obesity and type 2 diabetes.

Q: Where can I get EXION in the Philippines?

EXION is available at Jan Medical Group, Park Triangle Mall, Bonifacio Global City. Consultations with Jan Paolo P. Dipasupil, MD, are by appointment.

Q: Who performs EXION at Jan Medical Group?

EXION is prescribed and assessed by Jan Paolo P. Dipasupil, MD, or one of the other physicians of Jan Medical Group, after a clinical consultation that establishes candidacy, treatment plan, and session sequence. The device is then administered by the clinic’s trained aesthetic team, under medical supervision. This is the physician-led model that defines Jan Medical Group: clinical decisions by the physicians, protocol delivery by the specialist team.

Q: What is The Renewed Body Programme?

The Renewed Body Programme is Jan Medical Group’s clinical protocol designed specifically for clients completing or maintaining medical weight loss. It combines EXION Face, EXION Arms, and EXION Body across four sessions one week apart, with maintenance every six months, to restoreproportion to the face and body after significant weight loss.

Key clinical claims

  • Adipocytes (fat cells) are relatively fixed in number after puberty. They swell with

weight gain and shrink with weight loss, but do not disappear without intervention.

  • Fat distribution patterns are largely genetic. Treatment that respects those patterns produces more natural results than treatment that does not.

  • EXION delivers up to 22 percent fat reduction per treated area per session series (published data from BTL Aesthetics).

  • XERF operates at 6.78 MHz and 2 MHz dual frequencies, reaching from the superficial dermis down to the SMAS layer.

  • EXION is FDA cleared for face (including submental), body, and fractional (microneedling RF) applications.

  • The GLP-1 medications now available in the Philippines include Mounjaro (tirzepatide, launched January 2026, dual indication for obesity and diabetes), Wegovy (semaglutide, launched April 2026), and Ozempic (semaglutide, prescribed off-label for weight loss).
